Print off this order form, include your check and send to GWHS Class of 1981, P.O. Box 207, Clarendon Hills, IL 60514. If you have questions, send them to glenbardwest@earthlink.net. We have to order these items, so be patient!

Orders are due on 11/12. If you are close to the deadline when you send in your order, send an email to glenbardwest@earthlink.net saying you have sent money so we include you when we reorder.

Notes:

1. We have adjusted the prices slightly to include mailing supplies, shipping costs and the additional charge for ordering more of each item in odd quantities.

2. If you signed up on the sign up form and paid the night of the reunion, you will be getting a booklet for free in the mail soon.

3. Those who attended the reunion should have received a packet with what you ordered (booklet, video, t-shirt). If you did not receive what you ordered, let us know. Packets are also going out to those who ordered but didn’t attend.

4. We had planned to offer the grade school pictures for people to order, but Eric Friedemann has offered to print and mail photos out to everyone in the pictures. Eric is also generously making reprints of many of his pictures from the weekend and will be sending those out to everyone as well. We will have his pictures up on the website for you to look through soon - he will be sending pictures out in the next few weeks. This project will take quite a bit of time and effort, and we all appreciate it! Thanks Eric!
